is currently accepting new patients. Appointment times are based on medical need and patient convenience. All new patients should arrive 30 minutes prior to their appointment time.

Making an Appointment

To schedule an appointment please call, 870-247-9499 or email whfmclinic@whitehallfamilymedicine.com and a scheduler will try to schedule your appointment within the time frame you requested. We make every effort to see you in a timely manner. 

If you are calling to make an appointment for an annual physical please tell the scheduler so that the appropriate amount of time can be scheduled for your visit, and so that you can receive important instructions.

At the time of your appointment please bring:
  • Photo ID
  • Insurance cards
  • New patient forms
  • List of current medications
  • Co-payments and deductibles
Our providers see patients by scheduled appointment. Please call our office as soon as possible to cancel an appointment. We understand emergencies do happen, please make sure to alert the scheduler when calling as to the reason you are cancelling, especially if it is less than 24 hours before the appointment. Should you miss 2 or more appointments you will be seen on as a work in appointment only.

Co-pays, Co-Insurance, and Deductibles are due at the time of service.   We encourage you to review your coverage plan or contact your insurance for any specific coverage questions.

Self-Pay patients are accepted. Please call or email our office for details.

Existing Patients

Existing patients should arrive at the clinic 15 minutes prior to their scheduled appointment. If any of your patient information has changed since your last visit, including your insurance, make sure to note that information on our Information Change Form. If your insurance has changed, please bring your new insurance card with you to the clinic.

  • Be prepared to pay all co-pays and deductibles

Canceling an Appointment

If you need to cancel an appointment, please call at least 24 hours in advance or call 870-247-9499 or email whfmclinic@whitehallfamilymedicine.com.

Payments for Services?

Payment is requested at the time service is rendered unless other arrangements have been made prior to your appointment. We accept cash, check, debit and all major credit cards. We will be happy to file insurance claims, but co-pays and deductibles that have not been met are due at time of service. Please be prepared to show your insurance card EACH visit.

Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some insurance companies pay fixed allowances for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, coinsurance, or any other balance not paid for by your insurance.

Hospitalization

Our physicians do not admit patients to the hospital, but work with Jefferson Regional Medical Center, highly qualified hospitalists. A hospitalist is a physician who sees patients in the hospital on behalf of family practice physicians.

If you require hospitalization, Doctors Coleman and Ramiro will contact the hospitalist on call to explain your condition and gain admission. The hospitalist will closely monitor you in the hospital – order tests, medications and necessary treatment – while keeping your physician informed. Once discharged from the hospital, care will be provided by your regular physician.

Prescription Refills

Patients who take critical prescription medications are advised to adhere to the schedule of lab and office visits recommended by their doctor. Please bring your medications or a list of them, to your office visit and ask your doctor to write refills for any medications you anticipate needing.

Patients who have been seen by the doctor at appropriate intervals may call for refills on some prescriptions (antibiotics excluded). Please call your pharmacy 2 - 3 days prior to running out of medication so the pharmacy can fax or electronically transmit a refill request to the doctor, or you may call the clinic. Please give 48 hours’ notice for refill requests. Requests received after 2 p.m. will be processed within the next two business days.

Patient Messages

Our goal is to return your message the day it is received depending on the time received and clinic schedule. Please allow 24 hours for messages to be returned.

Electronic Medical Records System

We are in the process of implementing an Electronic Medical records system to enhance the quality of care we provide to our patients. During this time of implementation you may experience a longer than normal wait time to see the physician.

Motor Vehicle Accidents

We treat motor vehicle accidents, but due to the bureaucracy involved in filing the automobile insurance we require you to pay your visits in full. We will provide you with a super bill which will show visit type, diagnosis information and amount paid that you can present to the insurance company for reimbursement.

Motor vehicle accidents are not paid by private or government insurance plans.

Workers Compensation

Our physicians treat workers compensation injuries with prior approval from the companies workers comp insurance carrier or the employee's company.
