In a life-threatening emergency call 911 or go to the closest emergency department right away.
Prescription Refill Requests
Patients who take critical prescription medications are advised to adhere to the schedule of lab and office visits recommended by their doctor. Please bring your medications or a list of them to your office visit and ask your doctor to write refills for any medications you anticipate needing.
Patients who have been seen by the doctor at appropriate intervals may call for refills on some prescriptions (antibiotics excluded). Please call your pharmacy 2 to 3 days prior to running out of your medication so the pharmacy can request a refill, or you may call the clinic. In addition, you may submit refill requests via our secure Patient Portal.
Please give 48 hours notice for refill requests. Requests received after 2 p.m. will be processed within the next 2 business days.
We do not call in antibiotics without seeing the patient.
Payment for Services
Payment is requested at the time service is rendered unless other arrangements have been made prior to your appointment. We accept cash, check, debit cards and all major credit cards.
We will be happy to file insurance claims, but co-pays and deductibles that have not been met are due at time of service. Please be prepared to show your insurance card at each visit.
Please note that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some insurance companies pay fixed allowances for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid for by your insurance.
Medical Records Request
Patients may request a copy of their medical records at any time. Please complete the Medical Records Request form on the Forms Page. Our medical records service will send your records to the address you provided on the form.
