Prescription Refill Requests

If you are taking critical prescription medications, please adhere to the lab and office visit schedule recommended by your doctor. During your visits, bring your medications or a list of them, and ask your doctor to write refills for any medications you may need.

For patients who have been seen by their doctor at appropriate intervals, refills for some prescriptions (excluding antibiotics) can be requested by calling your pharmacy or our clinic. To avoid running out of medication, please contact your pharmacy 2 to 3 days in advance so they can request a refill from us.

If you are prescribed controlled medications, please contact the clinic 5 days before your medication runs out. Do not call your pharmacy, as controlled medications must be sent electronically with a secure code from the provider’s cell phone. Examples of controlled medications include Xanax, Ambien, Clonazepam, and Hydrocodone.

Elation Patient Portal

Our new Elation Patient Portal provides a quicker way to request refills for your medications. If you have not signed up yet, please call our office at 870-247-9499 to request an invitation.

Important Reminders:

  • Please provide 48 hours notice for all refill requests.
  • Requests received after 2 p.m. will be processed within the next 2 business days.
  • We do not call in antibiotics without seeing the patient.

Payment for Services

Payment is requested at the time service is rendered unless other arrangements have been made prior to your appointment. We accept cash, check, debit cards and all major credit cards.

We will be happy to file insurance claims, but co-pays and deductibles that have not been met are due at time of service. Please be prepared to show your insurance card at each visit.

Please note that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some insurance companies pay fixed allowances for certain procedures, and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance or any other balance not paid for by your insurance.


Medical Records Request

Patients may request a copy of their medical records at any time. Please complete the Medical Records Request form on the Forms Page. Our medical records service will send your records to the address you provided on the form.
