We are writing to inform you of an important transition within our practice that will enhance our quality of care. We are adopting a new electronic health record system called Elation Health.

As healthcare providers, we continuously seek ways to improve our services and streamline our processes to serve you better, our patients. We made the change on

 May 8, 2024.

The new system, Elation Health, is a significant step forward in our commitment to your care. It’s designed to simplify workflows for our providers, allowing us to focus more on providing care and less on administrative tasks. This means more time and attention for you. It also enables us to access your medical records efficiently, coordinate your care effectively, and communicate with you seamlessly through the new patient portal (Elation Passport). These improvements ensure you receive the best possible care and make your healthcare journey more convenient and personalized.  

During this transition, there may be adjustments; we will send you forms you may have already completed in our old system. You need to complete the forms again, including your insurance information. The information in the new forms will flow into your health record for medicines, past medical history, problem list, past surgical history, allergy list,  Immunizations you may have outside of our clinic, insurance, and family medical history. We ask for your patience and understanding as we adapt to the new system.

Here are a few key points regarding the transition:

Protecting your privacy and securing your health information is paramount to us. The new HIPAA-compliant system employs the latest security measures to safeguard your data.

Enhanced communication. Through messaging capabilities through the Elation Passport app, you can communicate with us securely and efficiently. The patient portal is called the Elation Passport for Patients. You can find the app in your Apple app store. You can set up your account once you are sent an invitation email. Please call our office about the invitation.  Signing up lets you quickly access your clinic’s medical information. Over the next few months, we will transition all your information from our old system to the new one. If you do not see what you need in the new Elation Passport patient portal, please call our office, and we will gladly provide a hard copy. Once you receive the email invitation and set up your new Elation Passport, you can see your prescriptions, request refills,  see test results, and send questions to the clinic staff, and soon, you will be able to make appointments online. The new system is HIPPA-compliant and uses advanced security to protect your data.

IMPORTANT PORTAL NOTICE: Existing Patient Security Services Migration

Added security measures have been added to our patient portal to ensure your health information is protected. All patient portal users are required to update their login information to make the portal more secure.

For users who currently share an email address to access the portal, we ask that you establish unique email addresses, one email for each patient accessing the portal, with the exception of minor patients
